Our main goal is to keep patients at home and out of the hospital. Our nurses are adept with various kinds of critical patient care skills such as:

  • Medication teaching
  • Administration and teaching of injection medication
  • Closely monitoring any changes in diagnosis, medication or devices
  • Educating the patient on how to best cope with their condition
  • Observing bowel and urinary output
  • Overseeing the patient diet

Home Health Aide Services

Home health aides with specialized training can administer a client’s medication or check vital signs under the direction of a registered nurse or other medical professional. Qualified home health aides may also change bandages and dressings and help with medical equipment. Typically, when in-home clients need skilled health care, home health aides are trained as a certified nursing assistant (CNA) or Patient Care Assistant (PCA).

  • Medication monitoring
  • Grocery shopping
  • Closely monitoring any changes in diagnosis, medication or devices
  • Meal planning and preparation
  • Organization of schedules and appointments
  • Transportation to doctor’s offices and other appointments

Physical Therapy

Home-care physical therapy occurs when a physical therapist comes to your home to provide rehabilitation services. This type of service is reserved for patients who are unable to leave the house to attend PT. This can occur for a variety of reasons, including:

  • Your condition is acute or severe and leaving the home presents a health risk
  • Your functional mobility is limited and you cannot travel to therapy
  • You lack adequate transportation or access to transportation to get to your physical therapy
  • You simply desire a private PT session in your home

Most physical therapy services in the United States are covered by insurance, and certain rules apply to home-care physical therapy to be covered by your insurance carrier.
